Portal hypertension

문맥 고혈압(Portal hypertension)는 hepatic venous pressure gradient가 5mmHg 이상인 경우이다.

임상 양상

• 원인 질환
  • Variceal bleeding
  • Ascites
  • 간성뇌증
  • 비장 비대 및 비기능 항진증
  • Portal hypertensive gastropathy: indolent mucosal bleeding
  • schistosomosis(S. mansoni)

치료

• 문맥압 항진에 따른 합병증 치료가 주를 이룬다
• 수술적 감압
• TIPS
• nonselective beta blocker: propranolol or nadolol
  • 정맥류의 초출혈 및 재출혈 예방에 효과적
  • active bleeding 시 금기
• Portal hypertensive gastropathy: BB로 출혈 예방, PPI는 효과없음, BRTO
