Tetralogy of fallot

팔로사징후 (Tetralogy of Fallot, TOF)은 청색증이 나타나는 선천성 심질환으로 다음 4가지 특성을 가짐.

• Right ventricular outflow tract obstruction
• VSD
• Deviation of the origin of the aorta to the right so that it overrides the VSD
• Concentric RV hypertrophy

임상 양상

• 청색증: 출생시 나타탈 수 있으나 대개 생후 3~6개월경에 나타남
• Clubbing of fingers & toes, polycythemia
• Systolic thrill at the lower & middle left sternal border
• 청진소견
  • loud, single S2: aortic component만 들림(P2 감소)
  • long systolic ejection murmur at middle & upper left sternal border(Pulmonary stenosis에 의해 생김)
  • ejection click: severe form에서
• Chest X-ray
  • no cardiomegaly, pulmonary vascular marking 감소
  • boot-shaped heart

무산소 발작(anoxic spell, tet spell, hypercyanotic spell)

• 울거나 운동 후 청색증이 심해지는 현상
• RVOT obstruction에 의해 발생, 불분명한 murmur
• 치료: chest-knee position(SVR↑)->oxygen supply->IV morphine & fluid -> propranolol
