하지정맥류(Varicose vein)는 Chronic venous diseases에 속하는 질환으로 확장되고 구불구불하게 불거져 나온 지름 3mm 이상의 superficlal veins을 말한다. [1]

임상 양상
- 분류
- Primary varicose veins
- saphenous vein의 밸브 기능 및 구조 이상, intrinsic weakness of the vein wall, high intraluminal pressure(이런 환자의 1/2에서 가족력 존재)
- aging, pregnancy, hormonal therapy, obesity, prolonged standing
- Secondary varicose veins
- venous hypertension(deep venous insufficiency or deep venous obstruction)
- incompetent perforating veins
- Arteriovenous fistulas
- Primary varicose veins
- 보통 무증상으로 다리의 외관상 불편함 호소
- Superficial venous thrombosis 발생 가능
- 드물게 varicosity rupture, bleeding
- 오래 서 있을 경우 둔한 통증, 찌르는 듯한 통증, 압박감 등
- Varicose ulcer
진단
- Physical Exam: palpable great saphenous veins
- Venous duplex ultrasonography: 기본적 진단 도구, The Perthes test과 Brodie-Trendelenburg test에서 점차 대체됨
치료
- 보통 대증 치료: 탄력 스타킹(1st measure)[2], 오래서있지 않기, 다리 올리기
- Endovenous thermal ablation, sclerotherapy, surgery: 심하거나 지속되는 경우 등

- 미용, 보존적 치료에도 불응, varix bleeding, Superficial thrombophlebitis, Lipodermatosclerosis, Venous stasis ulcer
