Dystocia

난산(dystocia)은 분만 과정 중 자궁 수축력의 부족이나 지나침, 골반 구조, 태아의 요인으로 인해 나타난다.

자궁 수축력

• 수축력의 평가
  • Montevidieo Unit: 10분간 baseline 위로 증가된 자궁압 총합, 200MVU 이상이면 충분한 수축력,
• Latent phase
  • Hypertonic uterine contraciton:asynchronous한 과수축, 진행안되면서 통증 심함, Morphine등으로 sadation 및 tocolytics 투여, 효과없으면 C/S
  • 잠복기 지연(prolonged latent phase): 미분만부 20시간 이상, 다분만부 14시간 동안 지속될 경우 라고 하며 침상 안정 시킨다.
• Active phase
  • Hypotonic uterine contraction: synchronous 하게 수축하나 수축력이 약함, oxytocin, PGE2 투여하고 효과없으면 C/S
  • 활성기 자궁목 확장 지연(protracted active phase dilatation)Dilation이 초산부 1.2cm/hr, 다분만부 1.5cm/hr보다 느리다.
  • protracted descent: Station이 초산부 1cm/hr, 다분만부 2cm/hr보다 느림
  • Arrest disorder
    • Prolonged deceleration phase: 초산부 3시간, 경산부 1시간
    • Secondary arrest of dilatation(자궁 경관 개대 정지): 2시간 이상 확장 없음
    • Arrest of decent: 1시간 이상 하강이 없음
    • Failure of descent: 2nd stage나 deceleration phase에서 하강이 없음
• 치료
  • latent phase: best rest하며 경과관찰 할 수 있다
  • active phase arrest: 골반 내진하여 CPD 없으면 oxytocin 유도 분만, 있으면 C/S

• contracted pelvic
  • Vaginal delivery를 시도해보고 실패시 C/S

태아

Cephalopelvic disproportion(CPD)

• shoulder dystocia: McRoberts maneuver(다리를 산모 배쪽으로 누른다) 안되면 suprapubic을 누르면서 태아의 머리를 당긴다
• breech presentation: 프라하 수기, 모리세 수기
  • Frank breech: the baby's bottom comes first, and his or her legs are flexed at the hip and extended at the knees (with feet near the ears); (65–70%), 질식분만 시도 가능
  • Complete breech: the baby's hips and knees are flexed so that the baby is sitting crosslegged, with feet beside the bottom
• Transverse lie: C/S
