Postpartum hemorrhage

분만 후 출혈(산후 출혈, Postpartum hemorrhage)는 분만 3기 완료 이후(태반 만출 이후) 500ml 이상의 출혈이 있는 경우를 말한다.

분류

• Immediate postpartum hemorrhage
  • Uterine atony: Oxytocin, Ergonovine, Bimanual uterine compression
  • 심한 경우 수혈, Uterine artery embolization, hysterectomy or uterine artery ligation
  • Uterine inversion
  • placeta retention: USG로 진단, 잔류태반 제거
  • Adherent placenta: 수혈 및 hyterectomy or uterine artery ligation
  • birth canal injury: 자궁수축있음에도 계속 출혈시 의심, 눈으로 출혈 부위 꼭 확인, 심한 열상은 봉합
• Delayed postpartum hemorrhage
  • placenta retention
  • 태반 부착부위 퇴축 부전
• Late postpartum hemorrhage: 분만 24시간 이후부터 12주까지 발생한 출혈
  • 자궁의 불완전 퇴축: V/S stable시 자궁 수축제 투여 가능
